I received a bill from one of your providers after a visit to the Emergency Department. How do I pay my bill?
For dates of service prior to October 1, 2022: please refer to the phone number on your billing statement or call 813-874-5707.

For dates of service on or after October 1, 2022: Excelis Medical Associates contracts with a company, Logix Health, that specializes in emergency department coding and billing. For billing questions, please contact Logix Health at 1-855-691-9890, or go online to www.doctorpayments.com. To pay by mail, please address payment as follows:

I have a question about a prescription I was given when I was in the Emergency Department and/or I need a refill.
